Food allergy
Adult: 200 mg four times daily. May double the dose if effect is not satisfactory within 2-3 wk.
Child: 100 mg four times daily. May double the dose if effect is not satisfactory within 2-3 weeks.

Allergic rhinitis
Adult: 2.5 or 5 mg of a 2% or 4% soln is admin as drops or spray into each nostril 4 times daily.

Hepatic impairment: Dosage adjustment may be needed. 
Inhalation
Asthma prophylaxis
Adult: 20 mg four times daily as dry powder/nebulised soln or 10 mg four times daily as aerosol, increased to 6-8 times daily if necessary, reduced to 5 mg four times daily once asthma has been stabilised. Additional doses may be taken before exercise. 
Elderly: 
Renal impairment: Dosage adjustment may be needed. 
Hepatic impairment: Dosage adjustment may be needed. 
Ophthalmic
Allergic conjunctivitis
Adult: Instill 2 drops of a 4% soln 4-6 times daily or apply a 4% oint bid-tid. 
Elderly: 
Renal impairment: Dosage adjustment may be needed. 
Hepatic impairment: Dosage adjustment may be needed.
Contraindications Hypersensitivity.
Special Precautions Lactation, cardiac arrhythmias, hepatic or renal dysfunction.
Adverse Drug Reactions Nausea, headache, dizziness, unpleasant taste, joint pain and swelling, skin rashes, aggravation of asthma, pulmonary infiltrates with eosinophilia, urticaria. Bronchospasm, wheezing, cough, nasal congestion and throat irritation following inhalation of dry powder. Intranasal use: Transient irritation of nasal mucosa. Sneezing and epistaxis (occasional). Eye drops: Transient burning and stinging.
Pregnancy Category (US FDA)
Category B: Either animal-reproduction studies have not demonstrated a foetal risk but there are no controlled studies in pregnant women or animal-reproduction studies have shown an adverse effect (other than a decrease in fertility) that was not confirmed in controlled studies in women in the 1sttrimester (and there is no evidence of a risk in later trimesters).

Pharmacology
Cromoglycic acid inhibits degranulation of mast cells and therefore suppresses the release of histamine, leukotrienes and slow-reacting substance of anaphylaxis.
Absorption: Oral: Poorly absorbed from the GIT with only 1% bioavailability. Inhalation: 8-10% of a dose of fine powd is deposited and rapidly absorbed from the lungs. Intranasal: < 7% of the dose is absorbed. Ophthalmic: 0.03% is absorbed.
ATC Classification R01AC01 - cromoglicic acid; Belongs to the class of topical antiallergic preparations, excluding corticosteroids. Used as antiallergic agents. 
S01GX01 - cromoglicic acid; Belongs to the class of other ophthalmologic antiallergics. 
R03BC01 - cromoglicic acid; Belongs to the class of other inhalants used in the treatment of obstructive airway diseases, antiallergic agents.

Why is this medication prescribed?

Cromolyn Nasal solution is used to prevent and treat stuffy nose, sneezing, runny nose, and other symptoms caused by allergies. 

Cromolyn oral inhalation is used to prevent the wheezing, shortness of breath, and troubled breathing caused by asthma. It also is used to prevent breathing difficulties (bronchospasm) during exercise. It works by preventing the release of substances that cause inflammation (swelling) in the air passages of the lungs.

This medication is sometimes prescribed for other uses; ask your doctor for more information.

How should this medicine be used?

Cromolyn comes as a solution to use with a special nasal applicator. It usually is inhaled three to six times a day to prevent allergy symptoms. It is most effective when used before you come in contact with substances that cause allergies. If you have seasonal allergies, continue to use the drug until the season is over.

Follow the directions on the package or your prescription label carefully, and ask your doctor to explain any part you do not understand. Use cromolyn exactly as directed. Do not use more or less of it or use it more often than directed or prescribed by your doctor.

It may take up to 4 weeks for cromolyn to work. If your symptoms have not improved after 4 weeks, tell your doctor.

Cromolyn is used with a special applicator (Nasalmatic). Before you use cromolyn for the first time, read the instructions provided with the solution. Ask your doctor, pharmacist, or respiratory therapist to demonstrate the proper technique. Practice using the device while in his or her presence.

If you are to use the nasal spray, first blow your nose, and clear it as much as possible. Insert the applicator into a nostril. Sniff as you squeeze the sprayer once. To prevent mucous from entering the sprayer, do not release your grip until after you remove the sprayer from your nose. Repeat this process for your other nostril.

What special precautions should I follow?

Before using cromolyn, 

  • tell your doctor if you are allergic to cromolyn or any other drugs.
  • tell your doctor what prescription and nonprescription medications you are taking, including vitamins.
  • tell your doctor if you have or have ever had liver or kidney disease.
  • tell your doctor if you are pregnant, plan to become pregnant, or are breast-feeding. If you become pregnant while using cromolyn, call your doctor.

What should I do if I forget a dose? 

Use the missed dose as soon as you remember it. However, if it is almost time for the next dose, skip the missed dose and continue your regular dosing schedule. Do not use a double dose to make up for a missed one.

What side effects can this medication cause?

Cromolyn may cause side effects. Tell your doctor if any of these symptoms are severe or do not go away: 

  • itching or burning nasal passages

  • sneezing

  • headache

  • stomach pain

If you experience either of the following symptoms, call your doctor immediately: 

  • wheezing

  • increased difficulty breathing

In case of overdose, consult your doctor. If the victim has collapsed or is not breathing, consult local medical emergency services.

What other information should I know?

Keep all appointments with your doctor and the laboratory. Your doctor will order certain lab tests to check your response to this medicine.

Do not let anyone else use your medication. If you still have symptoms and need further treatment, consult your doctor.

It is important for you to keep a written list of all of the prescription and nonprescription (over-the-counter) medicines you are taking, as well as any products such as vitamins, minerals, or other dietary supplements. You should bring this list with you each time you visit a doctor or if you are admitted to a hospital. It is also important information to carry with you in case of emergencies.
