Description 

Calcium is a necessary mineral for life. In addition to building bones and keeping them healthy, calcium helps in blood clot, nerve conduction and muscle contraction. About 99 percent of body calcium present in bones and teeth. Each day, calcium is lost through skin, nails, hair, sweat, urine and feces, but human body cannot produce new calcium.

It’s important to try to get calcium from the food. When we don’t get enough calcium for body’s needs, it is extracted from bones.

The amount of calcium requirement/day depends on age and sex.

Calcium Supplementation

The amount of calcium needed from a supplement depends on the amount of calcium received from food. It is recommended  to get daily amount of calcium from dietary source first and only make up for any shortfall from supplement.

In general, no supplements are required that is not needed. Calcium supplements are available in a wide range of preparations (including chewable and effervescent tablets) and in different amounts. When choosing the best supplement some criteria should be followed:

  • Choosing branded supplements with proven reliability. Look for labels that state “purified” or have the USP (United States Pharmacopeia) symbol that met certain standards for purity and quality.
  • Paying attention to the amount of elemental calcium, which is the actual amount of calcium in the supplement, as well as how many doses to take.
  • Calcium is absorbed best when taken in amounts of 500 – 600 mg or less. This is the case when you eat calcium rich foods or take supplements. Getting calcium-rich foods and/or supplements in smaller amounts throughout the day, preferably with a meal is highly recommended. 
  • Taking most calcium supplements with food. Gastric acid that helps absorb most calcium supplements. The one exception to the rule is calcium citrate, which can absorb well when taken with or without food.
  • Starting with a smaller amount (new calcium supplement) to better tolerate it. When switching supplements, better to start with 200-300mg/day for a week, and drink an extra 6-8 ounces of water with it. Then gradually add more calcium each week.
  • Side effects from calcium supplements, such as gas or constipation may occur. If increasing fluids in your diet does not solve the problem, try another type or brand of calcium. It may require trial and error to select the right supplement.
  • Paying attention about possible drug interactions.
Indications

Calcium Carbonate is used for the treatment or prevention of calcium depletion in patients in whom dietary measures are inadequate. Conditions that may be associated with calcium deficiency include hypoparathyroidism, achlorhydria, chronic diarrhea, vitamin D deficiency, steatorrhea, sprue, pregnancy and lactation, menopause, pancreatitis, renal failure, alkalosis, and hyperphosphataemia. Calcium Carbonate is being used increasingly often to treat hyperphosphataemia in chronic renal failure as well as those on continuous ambulatory peritoneal dialysis (CAPD) and haemodialysis. Many patients are unable to tolerate sufficient doses for complete phosphate control and require additional measures such as stringent dietary phosphate restriction or relatively small doses of aluminium hydroxide. Calcium Carbonate containing preparations can provide short-term relief of dyspeptic systems but are no longer recommended for longterm treatment of peptic ulceration.

Dosage

Calcium Carbonate is always used orally and when used as an antacid the recommended doses for adults are equivalent to 540-2000 mg Calcium Carbonate per day, doses for children being half of those for adults.

As a dietary supplement, such as for the prevention of osteoporosis, 1250-3750mg Calcium Carbonate (500-1500mg elemental calcium) daily is recommended in general, but again this will need to be tailored to the individual patient depending on any specific disease such as Calcium deficiency, malabsorption or parathyroid function. In pregnancy and lactation the recommended daily dose of calcium may be as high as 1200-1500 mg. In chronic renal failure the doses used vary from 2.5 - 9.0 gm Calcium Carbonate per day and need to be adjusted according to the individual patient. To maximize effective phosphate binding in this context the Calcium Carbonate should be given with meals.

Use in Pregnancy/lactation & other stages

Calcium containing drugs have been widely used in pregnancy by way of oral calcium supplementation or antacid therapy. Calcium Carbonate can be used in lactating women as well.

Calcium carbonate has been extensively studied in children and infants with chronic renal failure and is both safe and effective.

In case of elderly patients with renal failure when calcium carbonate is taken constipation may be troublesome one for this group. For this reason, monitoring of serum calcium and phosphate is of course indicated for elderly patients. 

Contraindications

Hypercalcaemia and hyperparathyroidism, Hypercalciuria and nephrolithiasis, Zollinger-Ellison syndrome, Concomitant digoxin therapy (requires careful monitoring of serum calcium level). 

When hypercalcaemia occurs, discontinuation of the drug is usually sufficient to return serum calcium concentrations to normal. Calcium salts should be used cautiously in patients with sarcoidosis, renal or cardiac disease, and in patients receiving cardiac glycosides. 

Precautions Heart/kidney dysfunction, allergic condition.
Adverse Reaction

Belching, constipation. Overdose can occur hypercalcemia, can occur even milk - alkali syndrome, manifested as hypercalcemia, alkalosis and renal insufficiency.

Drug Interactions

Calcium Carbonate may enhance the cardiac effects of digoxin and other cardiac glycosides, if systemic hypercalcaemia occurs. Calcium Carbonate may interfere with the absorption of concomitantly administered tetracycline preparations and in chronic renal failure modification of vitamin D therapy may be required to avoid hypercalcaemia when Calcium Carbonate is used as the primary phosphate binder.

Why is this medication prescribed?

Calcium carbonate is a dietary supplement used when the amount of calcium taken in the diet is not enough. Calcium is needed by the body for healthy bones, muscles, nervous system, and heart. Calcium carbonate also is used as an antacid to relieve heartburn, acid indigestion, and upset stomach. It is available with or without a prescription.

This medication is sometimes prescribed for other uses; ask your doctor or pharmacist for more information.

How should this medicine be used 

Calcium carbonate comes as a tablet, chewable tablet, capsule, and liquid to take by mouth. It is usually taken three or four times a day. Follow the directions on your prescription or package label carefully, and ask your doctor or pharmacist to explain any part you do not understand. Take calcium carbonate exactly as directed. Do not take more or less of it or take it more often than prescribed by your doctor. When using this medicine as a dietary supplement, take it with food or following meals.

Chewable tablets should be chewed thoroughly before being swallowed; do not swallow them whole. Drink a full glass of water after taking either the regular or chewable tablets or capsules. Some liquid forms of calcium carbonate must be shaken well before use.

Do not take calcium carbonate as an antacid for more than 2 weeks unless your doctor tells you to.

What special precautions should I follow?

 Before taking calcium carbonate,
  • tell your doctor if you are allergic to calcium carbonate or any other drugs.
  • tell your doctor what prescription and nonprescription medications you are taking, especially digoxin, etidronate, phenytoin, tetracycline, and vitamins. Do not take calcium carbonate within 1-2 hours of taking other medicines. Calcium may decrease the effectiveness of the other medicine.
  • tell your doctor if you have or have ever had kidney disease or stomach conditions.
  • tell your doctor if you are pregnant, plan to become pregnant, or are breast-feeding. If you become pregnant while taking calcium carbonate, call your doctor.

What should I do if I forget a dose? 

If you are taking calcium carbonate on a regular schedule, take the missed dose as soon you remember it. However, if it is almost time for the next dose, skip the missed dose and continue your regular dosing schedule. Do not take a double dose to make up for a missed one.

What side effects can this medication cause? 

Calcium carbonate may cause side effects. Tell your doctor if any of these symptoms are severe or do not go away: 

  • upset stomach

  • vomiting

  • stomach pain

  • belching

  • constipation

  • dry mouth

  • increased urination

  • loss of appetite

  • metallic taste

What should I know about storage and disposal of this medication?

Keep this medication in the container it came in, tightly closed, and out of reach of children. Store it at room temperature and away from excess heat and moisture (not in the bathroom). Throw away any medication that is outdated or no longer needed. Talk to your pharmacist about the proper disposal of your medication.

In case of emergency/overdose 

In case of overdose, consult your doctor. If the victim has collapsed or is not breathing, consult local medical emergency services.

What other information should I know?

If this medicine has been prescribed for you, keep all appointments with your doctor so that your response to calcium carbonate can be checked. Do not let anyone else take your medicine.

It is important for you to keep a written list of all of the prescription and nonprescription (over-the-counter) medicines you are taking, as well as any products such as vitamins, minerals, or other dietary supplements. You should bring this list with you each time you visit a doctor or if you are admitted to a hospital. It is also important information to carry with you in case of emergencies.
