Indication(s) & Dosage

Oral route
Otitis media
Adult: Based on amoxicillin dose, 250-500 mg every 8 hr or 500-750 mg every 12 hr.
Child: Based on amoxicillin dose: 125-250 mg every 8 hr. Children weighing <40 kg: 20-40 mg/kg/day in divided doses every 8 hr. Infants <3 mth: up to 30 mg/kg/day in divided doses every 12 hr. 

Sinusitis
Adult: Based on amoxicillin dose, 250-500 mg every 8 hr or 500-750 mg every 12 hr.
Child: Based on amoxicillin dose: 125-250 mg every 8 hr. Children weighing <40 kg: 20-40 mg/kg/day in divided doses every 8 hr. Infants <3 mth: up to 30 mg/kg/day in divided doses every 12 hr. 

Skin and soft tissue infections
Adult: Based on amoxicillin dose, 250-500 mg every 8 hr or 500-750 mg every 12 hr.
Child: Based on amoxicillin dose: 125-250 mg every 8 hr. Children weighing <40 kg: 20-40 mg/kg/day in divided doses every 8 hr. Infants <3 mth: up to 30 mg/kg/day in divided doses every 12 hr. 

Upper and lower respiratory tract infections
Adult: Based on amoxicillin dose, 250-500 mg every 8 hr or 500-750 mg every 12 hr.
Child: Based on amoxicillin dose: 125-250 mg every 8 hr. Children weighing <40 kg: 20-40 mg/kg/day in divided doses every 8 hr. Infants <3 mth: up to 30 mg/kg/day in divided doses every 12 hr. 

Susceptible infections
Adult: Based on amoxicillin dose, 250-500 mg every 8 hr or 500-750 mg every 12 hr.
Child: Based on amoxicillin dose: 125-250 mg every 8 hr. Children weighing <40 kg: 20-40 mg/kg/day in divided doses every 8 hr. Infants <3 mth: up to 30 mg/kg/day in divided doses every 12 hr. 

Dental abscesses
Adult: Based on amoxicillin dose: 3 g as a single dose, followed by another dose 8 hr later. 

Acute uncomplicated urinary tract infections
Adult: Based on amoxicillin dose: 3 g as a single dose, followed by another dose 10-12 hr later.

Severe or recurrent respiratory tract infections
Adult: Based on amoxicillin dose: 3 g bid. 

Uncomplicated gonorrhoea
Adult: Based on amoxicillin dose: 3 g as a single dose with 1 g of oral probenecid. 

Prophylaxis of endocarditis
Adult: Based on amoxicillin dose: 2 or 3 g as a single dose. To be taken 1 hr before the dental procedure. 

H.pylori infection
Adult: Based on amoxicillin dose: 0.75-1 g bid or 500 mg tid. To be taken with metronidazole or claithromycin and a PPI or ranitidine bismuth citrate. 

Parenteral route
Susceptible infections
Adult: Based on amoxicillin dose: 500 mg every 8 hr. In severe infections, may increase to 1 g every 6 hr. Can be given via IM inj or slow IV inj over 3-4 minutes or IV infusion over 30-60 minutes. 
Child: <10 yr: 50-100 mg/kg/day in divided doses.
Reconstitution: Powder for oral suspension: Reconstitute with appropriate amount of water as specified on the bottle. Shake vigorously until suspended.

Contraindications Penicillin allergy.
Special Precautions History of allergy especially to cephalosporins, infectious mononucleosis, severe renal impairment.
Adverse Drug Reaction(s) Nausea, vomiting, diarrhoea, indigestion, rash and urticaria, candida superinfection.
Potentially Fatal: Anaphylactic reaction with CV collapse esp with parenteral use.
Drug Interactions Allopurinol may reduce renal tubular secretion of amoxicillin thus increasing the serum levels of amoxicillin. Concurrent use may reduce the efficacy of oral contraceptives.

Food Interaction None on absorption. But incidence and severity of GI effects may be reduced.
Storage Oral: Store below 25°C. Parenteral: Store below 25°C.
Pharmacology Clavulanic acid has a high affinity for and binds to certain Beta-lactamases that generally inactivate amoxicillin by hydrolyzing its Beta-lactam ring. Combining clavulanate potassium with amoxicillin extends the antibacterial spectrum of amoxicillin to include many bacteria normally resistant to amoxicillin and other penicillins and cephalosporins.
Distribution: Protein binding: About 25% (Clavulanic acid); about 18% (Amoxicillin). Amoxicillin distributes readily into most body tissues and fluids except the brain and spinal fluid.
Excretion: Half-life after oral admin: 1.3 hr (Amoxicillin); 1 hr (Clavulanic acid). About 50-70% of amoxicillin and 25-40% of clavulanic acid are excreted unchanged in urine during the 1st 6 hr after admin.
ATC Classification J01CA04 - amoxicillin; Belongs to the class of penicillins with extended spectrum. Used in the treatment of systemic infections.

Why is Amoxicillin-Clavulanic acid prescribed?

The combination of amoxicillin and clavulanic acid is used to treat certain infections caused by bacteria, including infections of the ears, lungs, sinus, skin, and urinary tract. Amoxicillin is in a class of medications called penicillin-like antibiotics. It works by stopping the growth of bacteria. Clavulanic acid is in a class of medications called beta-lactamase inhibitors. It works by preventing bacteria from destroying amoxicillin. Antibiotics will not work for colds, flu, or other viral infections.

How should Amoxicillin-Clavulanic acid be used?

The combination of amoxicillin and clavulanic acid may come as a tablet, a chewable tablet, and a suspension (liquid) to take by mouth or as I/V Injection. It is usually taken with a meal or a snack every 8 hours (three times a day) or every 12 hours (twice a day). To help you remember to take amoxicillin and clavulanate, take it around the same time every day. Follow the directions on your prescription label carefully, and ask your doctor or pharmacist to explain any part you do not understand. Take amoxicillin and clavulanic acid exactly as directed. Do not take more or less of it or take it more often than prescribed by your doctor.

Shake the liquid well before each use to mix the medication evenly.

The chewable tablets should be chewed thoroughly before they are swallowed. The other tablets should be taken with a full glass of water.

The 250 mg and 500 mg tablets of amoxicillin and clavulanic acid contain the same amount of clavulanic acid. Do not substitute two 250 mg tablets for one 500 mg tablet. The 250 mg regular tablet and the 250 mg chewable tablet contain different amounts of clavulanic acid. They also should not be substituted.

Take amoxicillin and clavulanic acid until you finish the prescription, even if you feel better. Stopping amoxicillin and clavulanic acid too soon may cause bacteria to become resistant to antibiotics.

Other uses for Amoxicillin-Clavulanic acid

Amoxicillin and clavulanic acid also is used sometimes to treat certain sexually transmitted diseases (STD). Talk to your doctor about the possible risks of using Amoxicillin-Clavulanic acid for your condition.

Amoxicillin-Clavulanic acid may be prescribed for other uses; ask your doctor for more information.

What special precautions to follow?

Before taking amoxicillin and clavulanic acid,
  • inform your doctor if you are allergic to amoxicillin, clavulanic acid, penicillin, cephalosporins, or any other medications.
  • inform your doctor what prescription and nonprescription medications, vitamins, nutritional supplements, and herbal products you are taking. Be sure to mention either of the following: allopurinol and probenecid. Your doctor may need to change the doses of your medications or monitor you carefully for side effects.
  • inform your doctor if you have or have ever had kidney or liver disease, allergies, asthma, hay fever, hives, or mononucleosis.
  • you should know that amoxicillin and clavulanic acid may decrease the effectiveness of oral contraceptives (birth control pills). Plan to use another form of birth control while taking amoxicillin and clavulanic acid.
  • inform your doctor if you are pregnant, plan to become pregnant, or are breast-feeding. If you become pregnant while taking amoxicillin and clavulanic acid, call your doctor.

What special dietary instructions to follow?

Unless your doctor informs you otherwise, continue your normal diet.

What to do if I forget a dose?

Take the missed dose as soon as you remember it. However, if it is almost time for the next dose, skip the missed dose and continue your regular dosing schedule. Do not take a double dose to make up for a missed one.

What side effects can Amoxicillin-Clavulanic acid cause?

Amoxicillin and clavulanic acid may cause side effects. Tell your doctor if any of these symptoms are severe or do not go away:
  • diarrhea

  • upset stomach

  • vomiting

  • mild skin rash

If you experience any of the following symptoms, call your doctor immediately:
  • severe skin rash

  • itching

  • hives

  • difficulty breathing or swallowing

  • wheezing

  • vaginal itching and discharge

  • yellowing of the skin or eyes

Amoxicillin and clavulanic acid may cause other side effects. Call your doctor if you have any unusual problems while taking Amoxicillin-Clavulanic acid.

In case of emergency/overdose

In case of overdose, consult your doctor. If the victim has collapsed or is not breathing, call local medical emergency services.

Symptoms of overdose may include:
  • stomach pain

  • vomiting

  • diarrhea

  • rash

  • hyperactivity

  • drowsiness

What other information to know?

Keep all appointments with your doctor and the laboratory. Your doctor may order certain lab tests to check your body's response to amoxicillin and clavulanic acid.

If you are diabetic, check your sugar while taking Amoxicillin-Clavulanic acid.

Do not let anyone else take your medication. Your prescription is probably not refillable. If you still have symptoms of infection after you finish the amoxicillin and clavulanic acid, call your doctor.

It is important for you to keep a written list of all of the prescription and nonprescription (over-the-counter) medicines you are taking, as well as any products such as vitamins, minerals, or other dietary supplements. You should bring this list with you each time you visit a doctor or if you are admitted to a hospital. It is also important information to carry with you in case of emergencies.
