Indication(s) & dosage

Amlodipine is a calcium channel blocker and may be used alone or in combination with other antihypertensive and antianginal agents for the treatment of:
ï‚· Hypertension
o Amlodipine is indicated for the treatment of hypertension, to lower blood pressure. Lowering blood pressure reduces the risk of fatal and nonfatal cardiovascular events, primarily strokes and myocardial infarctions.
ï‚· Coronary Artery Disease
o Chronic Stable Angina
o Vasospastic Angina (Prinzmetal’s or Variant Angina)
o Angiographically Documented Coronary Artery Disease in patients without heart failure or an ejection fraction < 40%

Adults
The usual initial antihypertensive oral dose of Amlodipine is 5 mg once daily, and the maximum dose is 10 mg once daily. Small, fragile, or elderly patients, or patients with hepatic insufficiency may be started on 2.5 mg once daily and this dose may be used when adding Amlodipine to other antihypertensive therapy.
Adjust dosage according to blood pressure goals. In general, wait 7 to 14 days between titration steps. Titrate more rapidly, however, if clinically warranted, provided the patient is assessed frequently.
Angina: The recommended dose for chronic stable or vasospastic angina is 5–10 mg, with the lower dose suggested in the elderly and in patients with hepatic insufficiency. Most patients will require 10 mg for adequate effect.
Coronary artery disease: The recommended dose range for patients with coronary artery disease is 5–10mg once daily. In clinical studies, the majority of patients required 10mg.

Children
The effective antihypertensive oral dose in pediatric patients ages 6–17 years is 2.5 mg to 5 mg once daily. Doses in excess of 5 mg daily have not been studied in pediatric patients.

Administration May be taken with or without food.
Overdosage Overdosage may cause marked peripheral vasodilation and hypotension.
Contraindications Known hypersensitivity to dihydropyridines.
Special Precautions Impaired liver or renal function, CHF, sick-sinus syndrome, severe ventricular dysfunction, hypertrophic cardiomyopathy, severe aortic stenosis. Caution when used in patients with idiopathic hypertrophic subaortic stenosis. Elderly, children. Pregnancy, lactation.
Adverse Drug Reaction(s) Headache, peripheral oedema, fatigue, somnolence, nausea, abdominal pain, flushing, dyspepsia, palpitations, dizziness. Rarely pruritus, rash, dyspnoea, asthenia, muscle cramps.
Potentially Fatal: Hypotension, bradycardia, conductive system delay and CCF.
Drug Interactions Increased metabolism with rifampin. Reduced hypotensive effect with calcium. Potentiates effects of thiazide diuretics and ACE inhibitors. Avoid combination with β-blockers in patients with markedly impaired left ventricular function. May increase serum levels of CYP1A2 substrates e.g. aminophylline, fluvoxamine, ropinirole. CYP3A4 inhibitors (e.g. clarithromycin, doxycycline, isoniazid, nicardipine) may increase the effects of amlodipine. Additive BP-lowering effects when used with sildenafil, tadalafil or vardenafil.
Pregnancy Category (FDA) Category C: Either studies in animals have revealed adverse effects on the foetus (teratogenic or embryocidal or other) and there are no controlled studies in women or studies in women and animals are not available. Drugs should be given only if the potential benefit justifies the potential risk to the foetus.
Storage Store at 15-30°C.

Pharmacology

Amlodipine relaxes peripheral and coronary vascular smooth muscle. It produces coronary vasodilation by inhibiting the entry of Ca ions into the voltage-sensitive channels of the vascular smooth muscle and myocardium during depolarisation. It also increases myocardial O2 delivery in patients with vasospastic angina.
Absorption: Well absorbed from the GI tract (oral); peak plasma concentrations after 6-12 hr.
Distribution: Protein-binding: 97.5%.
Metabolism: Hepatic: Extensive.
Excretion: Via urine (mainly as metabolites, 10% as unchanged); 35-50 hr (elimination half-life).
ATC Classification C08CA01 - amlodipine; Belongs to the class of dihydropyridine derivative selective calcium-channel blockers with mainly vascular effects. Used in the treatment of cardiovascular diseases.

Why this medication is prescribed for?

Amlodipine is used alone or in combination with other medications to treat high blood pressure and chest pain (angina). Amlodipine is in a class of medications called calcium channel blockers. It lowers blood pressure by relaxing the blood vessels so the heart does not have to pump as hard. It controls chest pain by increasing the supply of blood to the heart. If taken regularly, amlodipine controls chest pain, but it does not stop chest pain once it starts. Your doctor may prescribe a different medication to take when you have chest pain.

How this medicine should be used?

Amlodipine comes as a tablet to take by mouth. It is usually taken once a day. To help you remember to take amlodipine, take it around the same time every day. Follow the directions on your prescription label carefully, and ask your doctor to explain any part you do not understand. Take amlodipine exactly as directed. Do not take more or less of it or take it more often than prescribed by your doctor.

Your doctor will probably start you on a low dose of amlodipine and gradually increase your dose.

Amlodipine controls high blood pressure and chest pain (angina) but does not cure them. Continue to take amlodipine even if you feel well. Do not stop taking amlodipine without talking to your doctor.

Special precautions to follow

Before taking amlodipine,

  • tell your doctor if you are allergic to amlodipine or any other medications.
  • tell your doctor what prescription and nonprescription medications, vitamins, nutritional supplements, and herbal products you are taking.
  • tell your doctor if you have or have ever had heart failure or liver disease.
  • tell your doctor if you are pregnant, plan to become pregnant, or are breast-feeding. If you become pregnant while taking amlodipine, call your doctor.

Special dietary instructions to follow

If your doctor prescribes a low-salt or low-sodium diet, follow these directions carefully.

In case of missing dose?

Take the missed dose as soon as you remember it. However, if it is almost time for the next dose, skip the missed dose and continue your regular dosing schedule. Do not take a double dose to make up for a missed one.

Side effects of this medication

Amlodipine may cause side effects. Tell your doctor if any of these symptoms are severe or do not go away:

  • swelling of the hands, feet, ankles, or lower legs

  • headache

  • upset stomach

  • stomach pain

  • dizziness or lightheadedness

  • drowsiness

  • excessive tiredness

  • flushing (feeling of warmth)

Some side effects can be serious. The following symptoms are uncommon, but if you experience any of them, call your doctor immediately:

  • more frequent or more severe chest pain

  • rapid, pounding, or irregular heartbeat

  • fainting

If you experience a serious side effect, consult your doctor.

Storage conditions for this medicine

Keep this medication in the container it came in, tightly closed, and out of reach of children. Store it at room temperature and away from excess heat and moisture (not in the bathroom). Throw away any medication that is outdated or no longer needed.

In cases of emergency/overdose

In case of overdose, consult your doctor. If the victim has collapsed or is not breathing, consult local medical emergency services.

Symptoms of overdose may include:

  • dizziness

  • fainting

  • rapid heartbeat

Other information

Keep all appointments with your doctor. Your blood pressure should be checked regularly to determine your response to amlodipine.

Do not let anyone else take your medication. Ask your pharmacist any questions you have about refilling your prescription.

It is important for you to keep a written list of all of the prescription and nonprescription (over-the-counter) medicines you are taking, as well as any products such as vitamins, minerals, or other dietary supplements.
